Summary of Job:

The Integrated Behavioral Health Clinician (LICSW, LCSW, LPC) is embedded within the primary care teams and works directly with the team to assess individuals with behavioral health concerns to provide brief treatment services within the primary care setting.  The IBH Clinician will expand the teams’ abilities to treat people with chronic medical conditions that are complicated by lifestyle factors.  The Integrated Behavioral Health Clinician will also provide therapeutic interventions to a panel of patients.

Essential Job Functions:

  • Maintains an active presence on primary care clinical teams.
  • Participates in medical team huddles, staff meetings and trainings, and individual supervision sessions with coordinator/director as assigned.
  • Consults with primary care providers in real time to assist patients with psychosocial issues. This is frequently done as a “warm hand off” during hours of clinical operation. Informal consultations and referrals from the primary care providers happens at other times as well.
  • Assists the primary care provider in recognizing, treating and managing mental health and psychosocial issues and acts as a contributing member to the primary care team.
  • Triages both scheduled and immediate referrals from the medical teams, treating some with brief treatment/intervention and referring others to more appropriate resources or for further services within the care team
  • Advises the primary care provider about which patients are better served at the primary care setting and which should be referred for specialty services.
  • Provides brief treatment interventions for mild to moderate behavioral health concerns, as well as crisis intervention.
  • Demonstrates competence in brief assessment, crisis management, triage, diagnosis, as well as mental health therapeutic services within a brief solution-focused model.
  • Conducts patient intakes for Behavioral Health Services, focusing on diagnostic and functional evaluations and makes recommendations to the primary care provider concerning the patients’ treatment goals and plan.
  • Provides consultation and training to primary care providers to enhance their skill and effectiveness in the treatment of mental health problems.
  • Gives primary care providers timely feedback about the patient’s care, treatment recommendations, and progress via documentation in the patient’s electronic health record and verbal feedback as allowed by client and HIPAA.
  • Initiates follow-up to ascertain how patients are doing and to determine if any changes in treatment approaches are indicated.
  • Assists in the detection of “at risk” patients and in the development of plans to prevent worsening of their condition.
  • Monitors and coordinates the delivery of health services for patients as related to behavioral health care.
  • Documents in the patients’ electronic health record in a complete and timely manner.
  • Keeps the primary care providers fully informed of the patient’s needs and progress and works with the providers to formulate treatment plans.
  • Works, where indicated, to effect behavioral changes in patients with, or at risk for, physical disorders and helps them to understand healthier lifestyle choices.
  • Provides patients with self-management skills and educational information needed so they can be full participants in their own treatment and recovery.

  • Shows the ability to obtain complete history and full conceptualization of the patient’s problem within the time limited session(s), as measured by the quality of the written assessment and the timely case presentation to the team.
  • Demonstrates a thorough knowledge of effective treatment protocols for brief solution-focused interventions and necessary cultural competencies for treating children and adults with mental health, as well as chronic disease lifestyle management.

  • Follows the AIDET communication adopted by this institution.
  • Enters data into the Lab. Information System and control monitoring websites.

  • Medical Lab Scientist: This person must have a B.S. degree in an accredited medical technology program which includes one year of training in a school of medical technology approved by the American Society of Clinical Pathologists.   • Current American Heart Association or ASHI Advanced Cardiac Life Support (ACLS).
  • Current American Heart Association or ASHI Pediatric Advanced Life Support (PALS) or NAEMT EPC.
  • Current or must obtain within 6 months NAEMT PHTLS or ITLS Certification.
  • Current or must obtain within 1-year NAEMT AMLS or approved CRHD equivalent.
  • Ability to pass a CRHD approved physical agility test.
  • Ability to provide high quality, compassionate patient care for 911 calls or ground transports.
  • Ability to work independently with little direct supervision using time management skills.
  • High understanding of potential liability concerns for patient care errors, and quality control process.
  • Ability to interact with the public and co-workers under circumstances which require the application of specific knowledge, tact, diplomacy, and sound judgment.
  • Competency with Microsoft products, WATRS, and other windows and web-based applications.
  • Works well under pressure.
  • Excellent customer service skills.
  • Excellent written and oral communication skills.

Preferred Qualifications:

  • Experience in a high-performance EMS environment.
  • AHA BLS Instructor certification.

Castle Rock Hospital District Providers practice at our outpatient Rural Health Clinic where we have on-site lab and radiology. Castle Rock currently provides Family Practice and Pediatrics.

Castle Rock offers a great work schedule with no call. Our clinic is open Monday-Friday from 8:00 a.m. to 6:00 p.m.  There may be opportunities for student loan forgiveness through various resources in Wyoming.
