New patients can call 307-872-4500 option 1 to schedule an appointment.

We accept most major insurances, Medicare and Wyoming Medicaid. Please contact your insurance company to verify coverage.

Our Billing Department hours are Monday-Friday, 8:00 am-5:00 pm. Our billing staff is always available to answer any questions you may have about your account and can take credit card payments over the phone. Please call 307-872-4500 option 4.

Please bring your current insurance card, driver’s license, medication bottles and co-payment to your appointment.

Please arrive 15 minutes before your appointment. Patients who are checking in for Medicare physicals should arrive 30 minutes before their appointment.

Please allow 48 hours for a prescription refill. Please contact your pharmacy directly to initiate a refill request, even if there are no refills indicated on your medication bottle. The pharmacy will generate an electronic request to our clinic.

You may also request a prescription refill via the Patient Portal If you are not currently enrolled with patient portal, please call our office and inform the scheduler you would like to activate your Patient Portal account. After speaking with our staff, you will receive an email with a link so that you can finish the activation process.

One of the advantages of the patient portal is the convenience of accessing your personal health information at anytime. If you have an existing patient portal account, please click the following link and you can log into your patient portal account.

If you would like to set up your patient portal account, please contact us at 307-872-4500 or fill out the following information. Once we receive your information, please look for an email with a link to complete the activation process.

It is the general policy of Castle Rock Hospital District that animals are not allowed in the facility. Exceptions to this policy will be made for service animals when accompanying disabled patients or visitors. Companion animals are not service animals and are not permitted on CRHD property.

It is important that you come to your scheduled appointments. It is our policy that you cancel at least 4 hours prior to your appointment. If you do not, you may be subject to a $30 fee and we may not be able to schedule future appointments at CRMC. When patients do not provide adequate notice that they will be unable to keep an appointment it prohibits another patient needing care from being able to see one of our providers.
